If you are using blueprints (ie guns with preset attachments) it was bugging out on launch could still be the case. If you weren't before when making your gun make sure you use a base blueprint (furthest left when selecting a gun) I had this with a blueprint sniper but it's stopped now so I imagine it's just select people still having the issue. Blueprints are having issues, change the gun to a diff one, mess around with an attachments or two and then swap back to your main gun and build it, fixed the insurance slot for me whenever it got locked up or wasnt accurate. #4.
